You need a program called Save Wizard, which you can use to "resign" it from the OP's account to your own account. For a ton of games, Save Wizard can also add cheats to your games so it's well worth it. After that, just USB stick from PC to PS4. I have 2 links below to guides with some more info in them.

hey man thank you for this save. and yes you do get trophies about 10 or 12 cant recall it, u only need the save five the rest leave it out. for some may not work otherwise. and if u doubt me then check my ps id LIGHTNINGDARKESS on playstation network n see the trophies i got so far all of the are from this save.

I checked out your profile and it seems this save unlocks around 19 trophies, and including most of the collectibles, looting corpses and gaining the Allied Trust. I think you can even unlock more with this save.
